The following will determine a patient's eligibility for Hospice Care:

The patient's primary physician will certify that the patient has a terminal illness with a life expectancy of six months or less (based on past historical data for that particular disease).

The patient or their Power of Attorney has elected for comfort care, and does not wish for any further aggressive, curative treatments.
