Legacy Hospice offers:

Routine Care: Care provided in the patient's home.

Continuous Care: Care provided at the patient's bedside during brief periods of crisis for pain control and acute symptom management.

Respite Care: Allows for the patient to be placed in an inpatient setting for up to five days to allow families relief and prevent caregiver burnout. Legacy Hospice professionals will continue to visit and provide ongoing care and support during this period.

Inpatient Hospice Care: Twenty-four hour care for symptom management or during one's final days.

You matter to the last moment of your life, and we will do all we can , not only to help you die peacefully, but also to live until you die."--Dame Cicely SaundersWhat is hospice care?In its earliest days, the concept of hospice was rooted in the centuries-old idea of offering a place of shelter and rest, or "hospitality" to weary and sick travelers on a long journey. In 1967, Dame Cicely Saunders at St. Christopher's Hospice in London first used the term "hospice" to describe specialized care for dying patients. Today, hospice care provides humane and compassionate care for people in the last phases of incurable disease so that they may live as fully and comfortably as possible. Hospice is a philosophy of care. The hospice philosophy or viewpoint accepts death as the final stage of life. The goal of hospice is to help patients live their last days as alert and pain-free as possible. Hospice care tries to manage symptoms so that a person's last days may be spent with dignity and quality, surrounded by their loved ones. Hospice affirms life and neither hastens nor postpones death. Hospice care treats the person rather than the disease; it focuses on quality rather than length of life. Hospice care is family-centered -- it includes the patient and the family in making decisions. This care is planned to cover 24 hours a day, 7 days a week. Hospice care can be given in the patient's home, a hospital, nursing home, or private hospice facility. Most hospice care in the United States is given in the home, with family members or friends serving as the main hands-on caregivers. Because of this, a patient getting home hospice care must have a caregiver in the home with them 24 hours a day.Hospice care is used when you can no longer be helped by curative treatment, and you are expected to live about 6 months or less if the illness runs its usual course. Hospice gives you palliative care, which is treatment to help relieve disease-related symptoms, but not cure the disease; its main purpose is to improve your quality of life. You, your family, and your doctor decide together when hospice care should begin.One of the problems with hospice is that it is often not started soon enough. Sometimes the doctor, patient, or family member will resist hospice because he or she thinks it means you're "giving up", or that there's no hope. This is not true. If you get better or the cancer goes into remission, you can be taken out of the hospice program and go into active cancer treatment. You can go back to hospice care later, if needed. But the hope that hospice brings is the hope of a quality life, making the best of each day during the last stages of advanced illness.

During this difficult time, the last thing you should be confronted is a lack of control over who cares for your loved one.Legacy Hospice began accepting patients in July 2003. Since we are a small Hospice, our patients and their loved ones are known personally by our entire field staff of experienced, compassionate and professional individuals.The rapport our agency establishes with our patients and their loved ones is invaluable and has tremendous meaning to our staff and agency. Our entire staff is dedicated to supporting our patients and their loved ones during a very difficult time. We are committed to our patients and their loved ones needs every step of the way.Our staff ensures that our patients and their loved ones are fully involved in the care of the patient. We take the time to listen to the patient and their loved one for we realize that our patient may have no control over their disease process, but they are in full control of their care.

Aftercare

The Bereavement Aftercare process encompasses care and support provided by close family members, friends as well as the family's church congregation and pastor and Legacy Hospice Chaplains. At Legacy Hospice, our Chaplain's seek to sensitively supplement the care given by the patients individual pastor and church in addition to be available to a greater degree to families when these support systems are not in place. Beravement Aftercare is provided up to and in some cases beyond thirteen months for families that have lost loved ones. Our services are provided by anexperienced and compassionate Chaplain committed to the spiritual and emotional well being of our patients and their loved ones during the most stressful time of their lives.The following are Bereavement Services available through Legacy Hospice:•Personal visits for the purpose of encouragement, prayer and grief counseling•Funeral and Memorial Services planned and conducted at the families request•Cards and letters of condolence, comfort, support and feedback on an ongoing basis•Phone calls to check up on family's spiritual and emotional well being•Monthly encouragement cards•Referral to Community Grief Recovery Support Groups as needed/requestedNeeding Hospice Care, for so long, the residents of East Texas did not have a choice.Frankly, we at Legacy Hospice feel you deserve the right to make a choice in your Hospice provider.
